Content about symptoms, falls, medications, memory care, assisted living, nursing homes, home care, palliative care, rehabilitation, or other health topics is general educational information. It is not a diagnosis, treatment plan, prescription, triage service, or substitute for a licensed clinician who knows the person’s medical history.
No medication decision-making
Medication lists, reminders, discount resources, and questions to ask a clinician are organizational tools only. Do not start, stop, change, combine, split, or substitute medications because of information on this site. Ask the prescriber or pharmacist about interactions, side effects, missed doses, and medication changes.
No hospice or care-level determination
The care-planning questionnaire does not establish hospice eligibility, nursing-home eligibility, medical necessity, competency, capacity, disability, or the correct placement for a specific person. Those decisions may require licensed professionals, insurers, government programs, legal representatives, and the person/family to evaluate the full situation.
No benefit, insurance, or payment guarantee
References to Medicare, Medicaid, VA benefits, SNAP, LIHEAP, Lifeline, prescription assistance, tax relief, or other programs are general. Eligibility, covered services, payment amounts, provider participation, income/resource rules, and application requirements can change and vary by person and jurisdiction.
Provider listings are not endorsements
Appearing in the ZIP finder does not mean a provider has been independently inspected, recommended, credentialed, or endorsed by Senior Care Compass. NPI issuance does not by itself prove licensure or credentials. Confirm current licensing, services, insurance participation, availability, quality information, and costs directly with authoritative sources.
Public information can change
Government and provider datasets can be revised, delayed, incomplete, or contain errors. The site may temporarily fail to retrieve a source. Always verify time-sensitive information before acting on it.
HIPAA and personal health records
The site does not currently ask users to store medical records or create a personal health record. It does not represent that it is a HIPAA covered entity or business associate. If future features begin receiving identifiable health information in a legally regulated role, those features must be separately evaluated and accompanied by appropriate privacy/security controls and notices before launch.
